For non-UP applicants, please contact the Office of the University Registrar.
Download and fill out the Internal Shifting Application Form and the Student Directory Form (write the program you are applying to as your degree (BS) and major (BA/BAA) in this form). Take note that you must have completed at least 30 credit units (except PE, NSTP, Math 20, and other non-credit courses) to be eligible for shifting.
Request a Statement of Account (SOA) for the shifting application fee of Php30.00 at this link or make at request at VSB Room 105. You need the SOA in order to make a face-to-face payment (either at the UPD Cashier or at the UP VSB Library) or via online.
Pay the shifting application fee at the UPD Cash Office, UP VSB Library, or via online. Be sure to secure your official receipt or print your online payment confirmation.
Fill out the VSB Internal Shifting Application Online Submission Form. The form requires you to upload your Internal Shifting Application Form and Student Directory Form (refer to No. 1 above). Your shifting application will not be processed if the online form is not received. A copy of your responses to the form sent via email serves as the confirmation of your submission.
Submit the hard copy of your documents (from no. 1 above) at VSB Room 105, together with your payment's official receipt. Look for Ms. Teresa Bal.
You will be contacted via email if further documents will be needed from you. Make sure you submit both the documents in hard copy form and via the online submissions form. Otherwise, your submission will be considered incomplete and will not be processed.
The deadline for the submission of documents is on 20 June 2025, at 5:00PM.
